About the Program
The AstraZeneca Foundation’s CHANGE program is rooted in the belief that everyone deserves a fair and just opportunity to achieve their highest level of health. The program focuses on addressing social drivers of health and improving access to timely, high-quality healthcare, particularly for populations facing systemic barriers such as affordability, geography, transportation, language, and provider availability.
CHANGE grants are intended to provide general operating support for nonprofit organizations working in innovative, collaborative, and community-driven ways to improve access to care—during the grant year and beyond.
Geographic Eligibility
Applicant organizations must serve communities in at least one of the following states:
- California
- Delaware
- Maryland
- Massachusetts
- Texas
Organizational Eligibility Criteria
To be eligible, organizations must:
- Be a U.S.-based nonprofit with a 501(c) designation (or eligible public entity)
- Not be a 501(c)(3) with a 509(a)(3) designation
- Have total annual revenue of $12 million or less (most recent tax year)
- Have at least 3 years of programming improving access to quality healthcare
- (Organizations with fewer than 3 years must clearly justify the innovation)
- Work to improve healthcare access for populations experiencing health disparities
2026 Funding Priorities
Priority will be given to organizations that:
- Promote screenings, early detection, treatment, or continuity of care for:
- Cardiovascular diseases
- Renal diseases
- Respiratory diseases
- Immunologic diseases
- Cancers
- Serve low-income and/or rural or remote communities
- Address healthcare access barriers such as transportation, affordability, language, or care coordination
Focus Areas & Eligible Activities
Funded organizations may support activities including (but not limited to):
- Access to healthcare services (screenings, prevention, wellness)
- Care coordination (community health workers, case managers, health coaches)
- Mitigating access barriers (transportation, language access, literacy, affordability)
Additional preference is given to organizations that:
- Use community-engaged and innovative approaches
- Build or expand cross-sector partnerships or coalitions
- Break down institutional silos to improve care access
- Engage community members in governance or decision-making
- Demonstrate capacity to measure impact and outcomes
- Show a clear plan for long-term sustainability
Ineligible Uses & Exclusions
The Foundation does not fund:
- Capital campaigns or building projects
- Research or clinical trials
- Medical education or degree/licensure training
- Individuals or for-profit organizations
- Endowments, journals, or advertising
- Political or lobbying activities
- Religious organizations not open to the general public
- Organizations that discriminate on protected characteristics
Finalist & Grant Requirements
Selected finalists must submit additional documentation, including:
- IRS Form 990 and audited (or unaudited) financials
- Key staff and leadership roles
- Target number of individuals served
- Short- and long-term access to care goals
Awardees must:
- Participate in a Community Advisory Board (CAB) with monthly virtual meetings
- Submit regular progress and evaluation reports
- Obtain written approval for major changes to scope, budget, or objectives
