Eligibility

Applicants must:

  • Be a nonprofit community organization based in Michigan
  • Demonstrate measurable programmatic and health outcomes
  • Submit a required concept paper form prior to full application

Blue Cross Blue Shield of Michigan employees and their immediate family members, as well as employees of BCBSM affiliates and their families, are ineligible.


Matching Requirements

Matching funds are required based on:

  • Organizational budget size
  • Percentage of Medicaid and uninsured patients served

Note: The applicant organization cannot serve as its own match.

For nonprofits with budgets under $10 million:

  • <30% Medicaid/uninsured population: 25% match required
  • >30% Medicaid/uninsured population: 10% match required

For nonprofits with budgets over $10 million:

  • <30% Medicaid/uninsured population: 60% match required
  • >30% Medicaid/uninsured population: 25% match required

Application Process

The Foundation uses a concept paper first process:

  1. Submit a concept paper form
  2. Selected applicants are invited to submit a full application

The updated process is designed to improve efficiency for both applicants and Foundation staff.

Applicants are encouraged to review the Foundation’s “What We Fund” and FAQ sections before applying.
