The institution, association, or cooperative must have a legitimate fellowship committee or similar selection body responsible for determining the fellowship recipient.

Important Applicant Requirement
Do not include the name of a potential fellow in the grant application.
Johnson & Johnson awards the grant to the institution rather than an individual, and the institution must maintain control over the selection of the fellowship beneficiary.

5. Faculty & Program Leadership
Identify the organizational leadership and faculty responsible for administering the fellowship.
6. Fellow Evaluation Plan
Describe how the institution will measure fellow participation, learning, development, and outcomes.
7. Budget
Provide a detailed budget and justification for the requested funding.
8. Organizational Qualifications
Demonstrate the organization’s oncology, nursing education, workforce development, research, or fellowship training expertise.
Reporting Requirements
At the conclusion of the fellowship, funded organizations will be expected to provide a report describing the program’s overall impact.
The report should also include feedback from the fellow who completed the program.
Organizations should therefore develop an evaluation process capable of documenting outcomes throughout the 12-month fellowship.
