Organizations applying for support must submit:
- A completed Donation Request Form
- A copy of the organization’s current mission statement
- Primary purpose designation
- Program description and/or organizational literature
- A properly completed W-9 form
The Donation Request Form is available at any Sturdy Savings Bank branch location.
Applications should clearly include:
- Description of the organization (history, purpose, and proposed project)
- Statement of need
- Primary purpose of the donation request
