Organizations must submit requests through The Bank of Elk River’s online Donation Request Form. Applicants should be prepared with:

  • Complete organizational contact information
  • Organization Tax ID / EIN
  • Impact information related to the organization or request

  2. Review and update pre-populated information
  3. Add your organization if it is not already listed
  4. Complete all remaining required fields and submit
