- Project description and alignment with Foundation pillars
- Updated financial statements
- Project budget (if applicable)
- Other funding sources (committed and pending)
- List of Board of Directors
- IRS determination letter confirming 501(c)(3) status

If approved, organizations may not reapply until the same quarter of the following year.
If declined, organizations may reapply in the quarter following their previous application.
Ineligible Requests
The Union Home Foundation does not provide grants to:
- Individuals or individual fundraising efforts
- Political campaigns, parties, or groups
- Religious organizations
- Organizations that discriminate based on protected characteristics
- Disease-related causes
