About the Funder
Wellpath is a national provider of healthcare services in correctional and secure settings. The Wellpath Cares Foundation supports nonprofit organizations working in communities where Wellpath provides correctional healthcare, with a particular emphasis on improving outcomes for justice-involved individuals.
The Community Impact Grant program was launched in October 2026 to provide targeted funding for local organizations addressing barriers that individuals may encounter during and following reentry into the community.

Geographic Eligibility

Important: Presence in one of these states does not necessarily make every organization in that state eligible. Applicants should confirm that Wellpath provides correctional healthcare services in the specific jurisdiction or community they serve before applying.

Why This Opportunity Is a Good Fit
This opportunity is particularly well suited for smaller and community-based nonprofits that:
- Serve people returning to the community after incarceration
- Provide employment readiness, transportation, housing, mentoring, or transitional support
- Work to reduce recidivism
- Provide substance use disorder treatment or recovery services
- Operate in a community where Wellpath provides correctional healthcare
- Have a defined project or service that can demonstrate measurable results
With awards starting at just $250 and reaching $10,000, the program may be especially accessible to smaller organizations seeking funding for targeted services rather than large-scale programs.
