Application / Request Process

Organizations requesting support must:

• Complete a Donation Request Form
• Attach supporting documentation, including:
  • Event or program materials (if applicable)
  • IRS 501(c)(3) tax determination letter (if applicable)

Completed requests are submitted to the Donations Committee for review.
