Eligibility & Key Requirements
To be considered, applicants must:

- Submit a complete online application with all required attachments
- Apply with sufficient lead time for review
Required Application Attachments
Applicants must have the following documents ready before starting the application (the form cannot be saved once started):
- 501(c)(3) IRS determination letter
- Signed W-9 form
- Sponsorship levels and a donation request letter
- LMI verification documentation, such as:
- Annual report or brochure showing income demographics served
- Letter from Executive Director (on letterhead) stating % of LMI individuals served
- Advertising specifications (if sponsorship includes ads), including deadlines and file formats
- Details for in-kind donation requests (e.g., raffle items, branded items, tickets, quantities)
