  • EIN/Tax ID
  • Organizational history, mission, and staffing
  • Board of directors list
  • Project summary and workforce impact
  • Job description (if funding new staff roles)
  • List of additional funding sources

Required Attachments:

  • Project budget with evaluation plan
  • Most recent audited financials or three years of Form 990s
  • Current operating budget
  • IRS 501(c)(3) determination letter
