Eligibility & Requirements

To be considered for funding, applicants must provide services in one or more states where Frankenmuth Insurance operates:

Illinois
Indiana
Kentucky

Ohio

Tennessee
Virginia
Wisconsin

Eligible applicants must be one of the following:

A tax-exempt organization under Internal Revenue Code Section 501(c)(3), classified as other than a private foundation under Code Section 509(a)
A governmental unit described in Code Section 170(c)(1), provided the grant is used exclusively for public services

Applicants must complete the online Frankenmuth Insurance Foundation application form and provide required documentation, including:
